The newly released image features several familiar faces from the long-running medical drama alongside newer cast members, but Pompeo is nowhere to be seen. After leading the series through its first 18 seasons, Pompeo reduced her on-screen role beginning with Season 19 in 2022 to spend more time with her family and pursue other projects. ABC shared the official Grey's Anatomy Season 23 cast photo on Instagram on July 24, bringing together returning stars Chandra Wilson, Debbie Allen, James Pickens Jr., Camilla Luddington and Caterina Scorsone, along with newer cast members including Trevor Jackson, Adelaide Kane, Niko Terho, Harry Shum Jr. and Anthony Hill.

Several fans questioned why Pompeo was missing from the promotional image despite Meredith Grey remaining one of the franchise's defining characters. While the new season announcement generated excitement, much of the conversation centered on why the show's most recognizable face was nowhere to be seen.

While she is no longer part of the full-time ensemble, the actress continues to play a significant role both on and off screen, with her involvement in the series extending far beyond regular on-screen appearances.

Ellen Pompeo is no longer a full-time series regular, but she has remained an important part of Grey's Anatomy. She continues to narrate the series, serves as an executive producer and has appeared in multiple episodes each season since scaling back her role. According to Deadline, that arrangement is expected to continue in Season 23 as well.

"I have been doing it for 20 years, so it was time to step away...I am very lucky to be able to get to work sometimes and take time off. I have a nice balance in my life." Reflecting on her decision during an April 2025 interview with El País, Pompeo explained why it was the right time to step away from the demanding schedule.

Season 23 will also mark a new chapter for the series following the departures of longtime cast members Kevin McKidd and Kim Raver after last season. In separate statements shared by Variety, both actors expressed gratitude for their years on the hit medical drama while looking ahead to future opportunities.

While Meredith Grey may not be featured in every promotional image, Pompeo's connection to Grey's Anatomy remains firmly intact. Her continued work behind the scenes and recurring on-screen appearances suggest that although her role has evolved, she is still very much a part of the world she helped define.
