Eva Celia portrays Karina, Sadimin’s new wife, while Mikha Tambayong plays his daughter, Kenes, caught in the ensuing turmoil. Damara, notably, had previously appeared in Lovely Man and The Big 4, Celia in The Golden Cane Warrior and The Shadow Strays, and Tambayong in Buffalo Boys and Fallin' in Love.

The supporting cast of The Elixir adds depth to the film’s unfolding chaos. Dimas Anggara portrays Rudi, Kenes’ husband (Crazy, Stupid, Love), while Marthino Lio plays Bang, Sadimin's irresponsible son (The Big 4). Ardit Erwandha (The East) and Claresta Taufan (On Your Lap) play Rahman, a police officer, and Ningsih, his partner, respectively. The Elixir, strikingly, comes from an experienced horror filmmaker.
