10 Times Hollywood Actors Almost Ruined Their Body for Extreme Physical Transformations in Movies
Hollywood actors have always pushed themselves to their physical limits while playing roles in movies. From excessive weight gain or loss to strict diets, the journey has been tough, just to make their performances realistic and memorable. But some actors later confessed that these dramatic changes led to injuries, exhaustion, and long-term health issues, proving that extreme movie makeovers can sometimes leave a painful physical impact.
Some of Hollywood’s most celebrated performances came only after actors pushed their bodies far beyond the safe physical limits.
1. Christian Bale — The Machinist

Christian Bale’s transformation in The Machinist is famous for its extreme and dangerous nature. He lost about 60-63 pounds to play Trevor Reznik, eating only one can of tuna and an apple a day. This transformation reportedly destroyed his body.
His dramatic weight loss had left his body weak, and his mind drained. Doctors and nutrition experts later warned that such rapid changes in the body could have resulted in serious long-term health problems. The transformation was even more shocking because Bale had to quickly regain drastic weight to prepare for Batman Begins.
Bale’s shocking transformation was not the only time a Hollywood actor pushed their body to dangerous extremes for a movie role.
2. Charlize Theron — Monster
Recently appearing in Netflix's Apex, a 27-year-old Charlize Theron gained 30 pounds for her movie Monster. Later, she opened up about the physical toll of her extreme transformation. She admitted that she had no regrets about winning an Oscar for Monster, but she would never get fat for a movie again.
Later, Theron said that it became a lot harder to recover from her physical transformation with age. The actress admitted that the process of gaining weight for the role impacted her health and energy levels for a long time. She said it was not as easy to lose the weight as it had been earlier in her career.
Theron’s drastic transformation for Monster proved that even Oscar-winning performances can leave a lasting physical toll behind the scenes.
3. Matthew McConaughey — Dallas Buyers Club
Matthew McConaughey underwent a major physical transformation for his role as Ron Woodroof in Dallas Buyers Club. He lost about 50 pounds for the role, going from about 188 pounds to 135 in several months. He gained praise, but it came at a heavy physical price and posed serious health risks.
He later said the drastic weight loss affected his physical health. He then said his eyesight started to fail, and his energy level dropped dramatically during the process. He also admitted that after walking a short distance or running a little bit, his legs locked up.

Along with her extreme weight loss, the actress had her hair cropped for the film - and it was a difficult, but necessary, sacrifice.”

5. Matt Damon — Courage Under Fire
In Courage Under Fire, Damon played a soldier suffering from addiction. But the extreme weight loss he underwent for the role almost did permanent damage to his health. Damon went on an unsupervised and dangerous regimen to get the look, resulting in severe medical complications.
He shed an incredible amount of weight in a very short period of time. He has since described the process as incredibly taxing. He further admitted to pushing his body to the limits of safety without professional medical oversight.
Damon later admitted that the dangerous transformation pushed his body beyond safe physical limits and left a lasting impact on his health, similar to Joaquin Phoenix.
6. Joaquin Phoenix — Joker
Joaquin Phoenix lost 51 pounds for Joker. He lost the weight carefully, with the guidance of a doctor. However, the strict calorie deprivation he endured affected his eating habits long after he reached his goal.
“Once you reach the target weight, everything changes,” he confessed to the Associated Press. He expressed how the extremities he willingly pushed himself to came with an obsession enough to develop a disorder.
Phoenix later revealed that the extreme transformation affected not only his body, but also his mental health and daily habits, something that Natalie Portman knew well about.
7. Natalie Portman — Black Swan
After going through an extreme transformation for her Oscar-winning role in Black Swan, Natalie Portman thought she literally was going to die, when she reminisced about the role for an interview with Entertainment Weekly in 2010. She went through a rigorous physical regimen, pushing her body to the brink of collapse. She then lost 20 pounds with a strict diet and training almost constantly.
Additionally, she had a restrictive diet with limited food options like carrots and almonds. She also talked about the isolating nature of the training. It was the first time she understood how an actor could get so wrapped up in a role that it could take them down.
Portman’s intense preparation for Black Swan showed how deeply some actors physically and emotionally immerse themselves into demanding roles.
8. Tom Hanks — Cast Away
Tom Hanks' transformation for Cast Away was so extreme that it resulted in him developing long-term health issues like Type 2 Diabetes. To truly represent a man on a deserted island, Hanks underwent a two-year production process. It included dramatic weight gain, significant weight loss, and a life-threatening injury.
He has said that he may have developed Type 2 diabetes because he has had to gain and lose weight so often for his roles. He also had a cut on his leg while shooting that got infected in a serious, life-threatening way. It then led to a three-day hospital stay and a delay in production.
Hanks’ painful experience during Cast Away is only a speck in the vast landscape of actors plunging themselves into physically preparing for their roles.
9. Jake Gyllenhaal — Nightcrawler
Jake Gyllenhaal tried to eat as few calories as possible for Nightcrawler, he revealed to Variety in 2014. He dropped 30 pounds, but not safe from a harrowing process. He revealed that the idea to make the character look gaunt was his own, perhaps also the reason behind how intensely he followed the drastic transformation.
“I knew if I was hungry that I was in the right spot,” Jake told Variety. He explained how even though the physique was bound to reflect physically, the mental and further inward transformation was far more intense for him, enough that it became a struggle.
Gyllenhaal later admitted that the extreme transformation actually did end up negatively affecting him both physically and mentally during the making of Nightcrawler.
10. Robert De Niro — Raging Bull
Robert De Niro’s transformation for Raging Bull was also remarkable. He deliberately risked his health to embody the boxer, Jake LaMotta. De Niro put on 60 pounds in four months to show LaMotta's physical degeneration, which took a heavy toll on his body. He also developed breathing problems.
The heavy, gasping breaths could be heard in the final scenes of the movie. Additionally, the sudden weight gain caused rashes on his legs from the rubbing.

What these actors did not get in return was safety from the scars for years to come. They proved that some celebrities would stop at nothing to give their fans the perfect performance, whether through losing/gaining excessive amounts of weight, exhaustion, or injuries.
