But in the process of working out the mystery, Daniels uncovers some very disturbing secrets, strange experiments, and baffling clues that start to blur the line between reality and paranoia. Traumatic memories, nightmares, and visions of his past haunt him throughout the film. 

At the end of the film, it is revealed that Daniels is really a patient at the asylum, Andrew Laeddis. It was a role-play cooked up by the doctors to implement an extreme therapy method to get him to finally admit the truth about his life and the tragic deaths of his family. For a moment, it seems Teddy has accepted reality, but then he talks as if he has slipped back into his delusion.

However, the final twist leaves audiences discussing whether Teddy really relapsed or if he made a conscious choice to pretend. His last line implies that he might actually know the truth, but no longer wants to bear the burden of guilt and pain of his memories. This emotional vagueness was what made the ending one of the most discussed psychological thriller endings in modern cinema.
